Contest begins Wednesday, May 19th, 2010.
Must be 18 years or older to qualify.
Must have validated driver's license to qualify.
Must fully complete all questions on the entry form in order to qualify.
Must be willing to sign a release form that allows us to follow your progress through our website, email newsletter and the media.
Must complete both the "Spirit Workshop", Chiropractic Assessment and start gym membership in order to be entitled to all of the additional certificates and prizes which include; hair cut and style, dinner and movie
